The redacted report released on Monday by the Northern Territory government found Michael Riches breached his statutory obligations, with five out of eight allegations against him substantiated and three unsubstantiated.
The former Independent Commissioner Against Corruption (ICAC) resigned earlier in May after being on paid leave for nearly a year as a workplace investigation took place.
Mr Riches was appointed in July 2021 and is understood to have been on a salary of around $500,000.
Allegations of inappropriate behaviour by Mr Riches were referred by then Chief Minister Eva Lawler in 2024 to the Commissioner for Public Employment who appointed Sarah Rey of Justitia Lawyers to conduct an investigation from July.
One of those interviewed for the investigation said Mr Riches "had a thing for pretty girls in the office" while another said "something is off about his approach to female staff".
Other witnesses described him as erratic, micromanaging, controlling, distrustful and awkward socially.
Mr Riches in his submissions to the investigator rejected the allegations, saying they were tenuous and "emblematic of trivial workplace grievances".
He described the office as having been in disarray and a shambles for some years prior to his arrival.
Two staff members who received alleged preferential treatment or special attention from Mr Riches declined invitations to participate in the investigation.
Mr Riches provided 13 references from former colleagues, prompting the investigator to review her findings of credibility and motivations of those who brought complaints.
But Ms Rey concluded there was a consistency of themes in the complaints and it was unlikely all 17 witnesses who came forward of their own accord had an axe to grind or acted in bad faith or colluded.
She found Mr Riches acted inappropriately when he gave "special and/or unwelcome attention to female employees in the office" and that he acted unreasonably when he gave preferential treatment to staff members.
The report was redacted to protect the privacy of those involved.
Mr Riches' resignation prompted the Inspector of the Northern Territory's ICAC to terminate further investigation into the matter as it was "no longer in the public interest and is an unnecessary expenditure of territory resources".
