Cameron William Leslie Clark, 33, on Friday appeared by video link in Brisbane Magistrates Court charged with the attempted murder of 20-year-old colleague Zoe Samson-Wood.
He is accused of slashing Ms Samson-Wood and two others with a large carving knife at Brisbane's Easts Leagues Club before being overpowered by patrons and staff on February 12.
Two earlier scheduled appearances were adjourned due to the accused undergoing medical treatment after police said he suffered a hand wound in the incident.
Four supporters of Ms Samson-Wood were in the gallery for the brief hearing before declining to comment outside court.
Legal Aid lawyers have been unable to speak with Clark since his arrest and did not know he had been released from the hospital, the court was told.
Clark did not apply for bail and asked for more time to seek legal advice.
"I'd like to adjourn it so I can prepare a little bit better," he told the court.
Clark was excluded from the leagues club about 2pm on the day of the alleged attack over complaints of "unwanted and unsolicited attention" to the waitress, police said.
About three hours later, he allegedly arrived at the club with the knife and attacked Ms Samson-Wood as dozens of people were dining.
"She was confronted in a hallway where she had little chance to escape," Detective Superintendent Andrew Massingham told reporters.
A 61-year-old chef at the leagues club - identified in court documents seen by AAP as Craig Geddes - came to her aid and was stabbed in the hip.
It gave Ms Samson-Wood time to escape to the main dining area where patrons and staff helped her, police said.
Clark allegedly attacked her again in the busy dining area, inflicting defensive wounds to her hands and 10cm cuts to her head and wrist.
Her sister Ruby Samson-Wood said her sibling was stabbed 15 times but put up a "hell of a fight".
She said Ms Samson-Wood had suffered nerve damage and would require reconstructive surgery.
A 41-year-old female patron suffered a hand laceration as up to eight people overpowered Clark, holding him down until police arrived minutes later.
Ms Samson-Wood's sister later paid tribute to the good Samaritans on social media.
"Thank you to anyone who helped, even if it gave Zoe an extra millisecond to get away, you saved her life," she wrote on Instagram.
Clark was also charged with stalking, acts intended to cause grievous bodily harm, unlawful wounding and going armed to cause fear.
After a brief hearing, he was remanded in custody to appear in court on March 16.
